Changing your Name
Blog : Marriage Extract application form:
You will need this form if you will be Changing your surname
Julianne Robertson Celebrant:
As a Marriage Celebrant on the beautiful Gold Coast, I have had lots of queries about what name the bride is known as when honeymooning. Sorry ladies, but when you travel abroad for your honeymoon (if it is straight after the marriage ceremony) you will need to travel on the name that is in your passport. But you can sign into hotels and such as Mr and Mrs ……………
Changing your surname
Changing your surname does not automatically happen after your wedding day. The certificate that you will receive on your wedding day is not enough to change your name.
You need to apply for a Marriage EXTRACT. I provide this form $37.00 plus $3.70 registered post for them to send to you. After the completion of your service, the marriage I as your celebrant have 14 days to send in all your documentation to the register of Births, Deaths and Marriages (BDM) (though I do it the next business day).
It then takes about 14 days for BDM to register the marriage.
then if you applied for the extract, 1 week to apply for the marriage extract, (looks similar to your birth certificate.
FROM OVER SEAS:????
OVER SEAS REGISTER: If you are wanting to register the marriage in another country, as some countries require the certificate to have an official Aposttile stamp affixed to same.
Please advice me of this as when I lodge the form I will require a letter from you asking for BDM to forward on to FDAT Foreign Affairs to received an Apostille stamp, along with an application form that can be downloaded from the FDAT site.
There is a fee applied to this. Please see website for fee details. Foreign affairs then send to your address over seas or to a nominated address in Australia.
IF YOU ARE APPLYING FOR YOUR OWN CERTIFCATE LATER THEN REMEMBER TO ASK FOR THE CERTIFCATE TO BE SENT TO FOREIGN AFFAIRS RE A LETTER AND MONIES, OR IT WILL BE SENT TO YOU THEN YOU WILL HAVE TO SEND IT BACK TO AUSTRALIA TO GET STAMPED.
IF YOU DECIDE TO APPLY LATER THEN
The following website has the form that you need to fill out to obtain your official marriage certificate
http://www.bdm.nsw.gov.au/PDF%20forms/03-MARRIAGE-0810.pdf
Only one of you need to apply for this, and if it is the bride, then all information must be completed in your maiden name. Once you have filled it in, you can either send it in to BDM or take it there yourself.
IDENTIY CHECK IF DOING ON YOUR OWN:
OF 100 POINT WILL BE REQUIRED. ALL DOCUMENTS TO BE WITNESSED BY A JP AND COPIES SENT ALONG WITH APPLICATION.
If you are sending it, you need to provide certified copies of the three identification categories found on the form. To certify your ID, you need to take them to a Justice of the Peace ( which most pharmacists are, and you also might find one in your local police station).
There are 3 types of marriage certificates.
The first commemorative, I supply that you sign, on the day
The second are the commemorative decorative certificates, and these are purely decorative and cannot be used for official business (they are really only if you want to put one in a frame on the wall) and
The third the marriage certificate extract you need to change our name.
After you have received your official certificate, you can go about changing your name. I found the following site that you can obtain a pack of information, which may be useful

Thanks Julianne
